Home Healthcare Registered Nurse (RN) Hospice Experience Required
We are seeking a compassionate and skilled Registered Nurse (RN) with experience in hospice care to join in providing high-quality, patient-centered care in the comfort of patients' homes. As a Home Healthcare RN, you will be responsible for delivering comprehensive, compassionate, and personalized care to individuals facing terminal illnesses. Your expertise in hospice care will be vital in ensuring patients' comfort, dignity, and quality of life during their final stages.
Key Responsibilities:
- Provide direct patient care to individuals in their homes, including assessment, care planning, and nursing interventions in accordance with the patient's needs.
- Coordinate and implement hospice care plans, ensuring all aspects of the plan are executed efficiently and effectively.
- Assess, monitor, and document patients' physical, emotional, and psychological status regularly, making updates to care plans as needed.
- Administer medications, wound care, and treatments as ordered, ensuring safe and effective care.
- Offer emotional and psychological support to patients and their families, providing education on the dying process and coping strategies.
- Collaborate with interdisciplinary teams, including physicians, social workers, chaplains, and other healthcare providers, to ensure holistic care delivery.
- Advocate for patient comfort by managing symptoms like pain and anxiety with compassion and professionalism.
- Educate patients and families about self-care techniques, end-of-life care, and hospice benefits, ensuring they understand the process and options available.
- Comply with all regulations and policies related to healthcare documentation, patient confidentiality, and patient safety.
Qualifications:
- Current Registered Nurse (RN) license in the state of [insert state].
- At least 1 year of clinical nursing experience in hospice care or a related field (home healthcare experience is a plus).
- Strong knowledge of hospice care principles, pain management, and symptom control.
- Excellent communication skills, both written and verbal, with the ability to communicate effectively with patients, families, and the healthcare team.
- Ability to work independently, demonstrate excellent judgment, and maintain a compassionate and professional demeanor.
- Current CPR certification required.
- Must have reliable transportation and be able to travel to patient homes within the designated service area.
Preferred Qualifications:
- Hospice certification (CHPN Certified Hospice and Palliative Nurse) preferred.
- Experience in palliative care or geriatric care is a plus.
- Familiarity with electronic health records (EHR) and home healthcare software.
