Hospice Home Health Licensed Practical Nurse (LPN)
We are seeking a compassionate, dedicated Hospice Home Health Licensed Practical Nurse (LPN). In this role, you will provide essential nursing services to patients facing terminal illnesses in their homes, focusing on comfort, dignity, and quality of life. As part of an interdisciplinary team, you will deliver compassionate care to patients and families, ensuring comfort measures, pain relief, and emotional support during the end-of-life journey.
Key Responsibilities:
- Patient Care & Assessments: Provide skilled nursing care to patients in their homes, including medication administration, wound care, vital sign monitoring, and symptom management.
- Pain & Symptom Management: Assist in managing patients' pain and other symptoms, ensuring comfort and quality of life, while adhering to the care plan developed by the physician and hospice team.
- Care Plan Implementation: Support the development and implementation of individualized care plans, closely following guidance from the RN Case Manager, physician, and hospice team.
- Patient & Family Education: Educate patients and family members on the patient s condition, medication regimen, comfort measures, and caregiving techniques to promote a positive experience during the end-of-life process.
- Emotional & Spiritual Support: Provide compassionate, empathetic support to patients and families, recognizing the emotional and spiritual needs of those dealing with end-of-life issues.
- Documentation & Reporting: Maintain accurate and timely documentation in patient records, documenting assessments, interventions, care plans, and patient responses in accordance with agency policies and regulatory requirements.
- Collaboration: Work as part of a multidisciplinary team, including physicians, social workers, chaplains, and other healthcare professionals, to ensure holistic, coordinated care for each patient.
- On-Call Support: Participate in on-call rotation as required to provide after-hours care and support to patients and families when needed.
Qualifications:
- Licensure: Valid, unencumbered LPN license in the state of Indiana.
- Experience: Minimum of 5 years of nursing experience. Experience in hospice, palliative care, or home health is preferred.
- Certifications: BLS (Basic Life Support) certification required. Certification in Hospice and Palliative Care (CHPN) is a plus but not required.
- Skills: Strong clinical skills in medication administration, wound care, and symptom management. Excellent communication and interpersonal skills to work effectively with patients, families, and team members.
- Personal Attributes: Ability to provide empathetic, compassionate care in challenging circumstances. Comfort with end-of-life issues and the ability to handle sensitive situations with respect and professionalism.
