Home Health Speech Therapist (SLP)
We are looking for a compassionate and skilled Home Health Speech Therapist (SLP) to join in providing high-quality, patient-centered therapy services in the home setting. The Speech Therapist will assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders in patients, helping them regain their independence and improve their quality of life. You will work closely with a team of healthcare professionals to develop and implement individualized care plans for patients in the comfort of their own homes.
Key Responsibilities:
- Patient Assessment & Evaluation:
- Conduct comprehensive assessments of patients speech, language, swallowing, and cognitive skills.
- Develop individualized treatment plans based on the assessment findings and patient goals.
- Reassess patients progress and modify treatment plans as necessary to meet evolving needs.
- Therapy & Treatment:
- Provide direct speech-language therapy services to patients in their homes, addressing a wide range of conditions including speech disorders, language delays, cognitive impairments, and swallowing difficulties.
- Develop and implement therapeutic activities and exercises tailored to individual patient needs.
- Provide education and guidance to patients and families on strategies to improve speech, language, and swallowing abilities.
- Collaboration & Care Planning:
- Collaborate with the interdisciplinary care team, including physicians, nurses, and other therapists, to ensure comprehensive care and treatment for the patient.
- Participate in case conferences and communicate regularly with the care team to update progress, address challenges, and adjust treatment plans as needed.
- Patient Education & Family Support:
- Educate patients and families on techniques to enhance communication and swallowing abilities, and how to manage any challenges at home.
- Provide counseling and emotional support to families to help them better understand the patient s condition and needs.
- Documentation & Compliance:
- Maintain accurate and up-to-date patient records, documenting assessments, interventions, treatment plans, and patient progress in compliance with regulatory and agency standards.
- Ensure documentation follows all state, federal, and HIPAA guidelines, meeting requirements for insurance and reimbursement.
- Professional Development:
- Stay current on best practices, emerging research, and new treatment techniques in speech therapy, particularly in home health settings.
- Participate in ongoing training and professional development opportunities.
Qualifications:
- Licensure & Certification:
- Current, active, and unencumbered Speech-Language Pathologist (SLP) license in the state of Indiana.
- Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) is preferred.
- BLS (Basic Life Support) certification required.
- Experience:
- Minimum of 5 years of experience as a Speech Therapist, preferably with experience in home health or a similar setting.
- Experience working with patients with various conditions such as stroke, dementia, neurological disorders, or swallowing disorders is preferred.
- Skills:
- Strong clinical assessment and treatment planning skills.
- Ability to work independently, manage a caseload, and prioritize patient needs.
- Excellent communication skills, both verbal and written, with the ability to educate patients and families clearly and empathetically.
- Knowledge of home health regulations and best practices in therapy services.

- Strong problem-solving skills and the ability to adapt treatment strategies based on patient needs and progress.
