Licensed Practical Nurse / LPN
We are seeking a Hospice LPN to provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse.
Responsibilities
- Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
- Evaluates the significance of assessment findings and communicates any pertinent information about the patient s clinical status and ongoing needs to others in a timely fashion
- Assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Demonstrates knowledge of medications and their correct administration based on age of the patient and patient s clinical position.
- Follows the seven (7) medication rights and reduces the potential for medication errors.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Functions as a team member demonstrating collaboration with, and responsibility to patient, physician, interdisciplinary team.
- Performs patient care responsibilities considering needs to the standard of care for patient s age.
- Articulates HHA grievance/concerns procedure.
- Articulates HHA adverse event/incident reporting procedure.
- Articulates organization s emergency management plan.
- Assists the physician and registered nurse to perform specialized procedures.
- Works as part of the interdisciplinary team.
- Completes patient documentation on time.
- Attends in-services and staff meetings as needed.
- Completes assigned training on time.
- Completes annual education requirements.
- Maintains patient confidentiality at all times.
- Performs other duties as assigned.
Qualifications
- Graduate of an accredited nursing program.
- Current IN state licensure as a LPN.
- Experience as an LPN commensurate with one of the following:
- One (1) year experience in an acute care setting within the last 24 months;
- Six (6) months home care experience within the last 12 months.
- Prior home healthcare experience is preferred.
- Valid state driver s license and reliable automobile, current automobile insurance and being willing to operate personal car(s) necessitated by nature of job.
- Current TB testing.
- Current CPR card.
