Job Description

Licensed Practical Nurse / LPN


We are seeking a Hospice LPN to provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse.


Responsibilities

  • Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
  • Evaluates the significance of assessment findings and communicates any pertinent information about the patient s clinical status and ongoing needs to others in a timely fashion
  • Assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patient s clinical position.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Functions as a team member demonstrating collaboration with, and responsibility to patient, physician, interdisciplinary team.
  • Performs patient care responsibilities considering needs to the standard of care for patient s age.
  • Articulates HHA grievance/concerns procedure.
  • Articulates HHA adverse event/incident reporting procedure.
  • Articulates organization s emergency management plan.
  • Assists the physician and registered nurse to perform specialized procedures.
  • Works as part of the interdisciplinary team.
  • Completes patient documentation on time.
  • Attends in-services and staff meetings as needed.
  • Completes assigned training on time.
  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Performs other duties as assigned.

Qualifications

  • Graduate of an accredited nursing program.
  • Current IN state licensure as a LPN.
  • Experience as an LPN commensurate with one of the following:
  • One (1) year experience in an acute care setting within the last 24 months;
  • Six (6) months home care experience within the last 12 months.
  • Prior home healthcare experience is preferred.
  • Valid state driver s license and reliable automobile, current automobile insurance and being willing to operate personal car(s) necessitated by nature of job.
  • Current TB testing.
  • Current CPR card.
